These forms are for:

• Making a privacy complaint to the University;
• Students seeking access to their student file;
• Staff seeking access to their staff file.

Please note  applications for access to student files should be directed to the relevant Faculty Office and applications for access to staff files should be directed to the HR Service Centre.
