Period of approval and annual report forms

How long is my approval valid for?

Ethics approval is given for a period of twelve months and can be renewed provided that a progress/annual report is returned to the Ethics Office. Failure to submit reports will result in withdrawal of consent for the project to proceed. A protocol may be renewed up to a period of four (4) years after which time a new application is required.

 


Where do I get an annual report from?

A progress/annual report is required by the Committee at the end of twelve months. A request for an annual report will be forwarded to the investigator by the Ethics Office. The report must provide details on:

(a) Security of records.
(b) The compliance with approved consent procedures and documentation.
(c) Compliance with other special conditions.
(d) Copies of the Participant Information Statement and the Consent Form which are being used by the investigators.
(e) At the completion of the project a Completion Report is required (see below "What if I complete my study before the reporting date?").

What if I complete my study before the reporting date?

If the study is completed prior to the reporting date the researcher must notify the Ethics Office and complete a Completion Report form.
