Complaints and queries clauses

Each Participant Information Statement must list a contact phone number and/or address for the research team for queries about the research.

Each Participant Information Statement must list at the end of the document the Ethics Office phone number, in the following form:
"Anyone with concerns or complaints regarding the conduct a research study can contact the Manager, Office of Ethics Administration, University of Sydney on (02) 8627 8175 (Telephone); (02) 8627 8180 (Facsimile) or gbriody@usyd.edu.au (Email)."

This should be in bold typeface of at least size 12 font.
