GUIDELINES FOR THE PREPARATION OF INFORMATION STATEMENTS AND CONSENT FORMS

The proposed process of providing information and obtaining informed consent from prospective research participants is a crucial factor in determining the ethical acceptability of a research proposal. The Ethics Committee must be satisfied that the researcher has provided the participant with information that a reasonable person would wish to know before making a voluntary decision to participate.

You can download examples of the various consent forms and information sheets discussed in this section of the web site by following the Sample Documents link in the quicklinks box on the left. These are in MS word6/95 format.
