Responding to an Incident Report

Upon receipt of an incident report the supervisor/manager of the activity is required to consult with those involved to investigate the circumstances and ensure that suitable corrective actions are taken to prevent any recurrence if possible.

A summary of the proposed actions must be emailled to within seven days of the incident notification. This response should include the incident report number and be copied to the relevant Head of Department and Departmental Safety Officer.

When deciding on suitable corrective actions the following risk controls should be considered:

  • Outsource the task
  • Use safer materials
  • Improve design
  • Replace/maintain equipment
  • Use a safety device
  • Establish safe working procedures
  • Provide training
  • Improve housekeeping
  • Improve signage
  • Use personal protective equipment (PPE)
  • Report a facility issue to Campus Infrastructure Services (CIS)
