Criminal Record Checks - for certain UAC commencing students

The University is required to have certain students complete a Criminal Record Check Consent form. The NSW Health Department requires students whose degree program includes a placement in a NSW Health Department facility to complete a consent form. This form will be issued at enrolment to students taking up a place in any of the following courses:

What you need to complete the consent form

While full information about the Criminal Record Check Consent form will be issued at enrolment, you can benefit by coming to enrolment with certain identification documents. If you are able to present these documents at enrolment, you will be able to finalise the completion of the Consent form on the day. Otherwise you will need to return with the required identification documents to enable the completion of the Consent form at a later date.

The Criminal Record Check Consent form requires you to satisfy a 100 point identification check. The following table provides you with some selected documents that you can bring with you to satisfy this check. There are many other documents that can be used. The full list will be presented to you on the form itself at enrolment.

Only original documents, or certified copies of documents are acceptable.
