Request for Exam Adjustments - Formal Exams On The Basis of Disability

Please fill in the sections below. Incomplete forms may result in a delay in implementing your Exam Adjustment request. This form must be submitted to the Disability Services office between the 5th and the 16th of October 2009. Late forms will only be accepted if extenuating circumstances are confirmed during an appointment with a Disability Services Officer.

Exam Adjustments

 

Exam Adjustments

You are only eligible to receive the same Exam Adjustments that you have had in the past. If you have not had Exam Adjustments at University of Sydney before, you must be assessed during an appointment with a Disability Services Officer before these conditions may be requested. If you are seeking different adjustments or changes to your existing adjustments, you will need to make an appointment to see a Disability Service officer. Please contact 8627 8433 to make an appointment prior to Oct 16th.
